Permanent makeup consent form: what it should include
Before any permanent makeup treatment, the client needs to know what to expect and confirm it in writing. A good informed consent form protects them and protects you. Here is what should not be missing, and how to manage it without paper.
What a consent form is for
The consent form records that the client received clear information about the treatment, the risks and the aftercare, and that they declared their state of health. If a dispute arises, it is the evidence of how you worked. That is why it should be signed before you start and kept in an orderly way.
What should not be missing
- Client and studio details, with the name of the person carrying out the treatment
- Description of the treatment, the area and the technique
- Risks, possible reactions and the result that can be expected
- Declarations on state of health and contraindications
- Instructions for before and after the treatment
- A separate authorisation for the use of photos
- Date and signature of the client, or of a guardian for minors
Treatment consent and privacy are not the same thing
Informed consent is about the treatment. The privacy notice is about personal data, which in this line of work includes health data and photographs. They are two separate documents and it is worth keeping them apart: that way a client can, for example, agree to the treatment but not to having their photos published.
How to have it signed without paper
With Pigmatia the client reads and signs directly on iPhone or iPad. The medical history is attached to the document, the wording can be customised, and for minors there is a guardian's signature. The consent becomes a PDF with your studio's details, ready to file or send, and it stays in the client's record.
When to renew it
A form signed years ago does not cover a new treatment. Have it signed again when the area or technique changes and when a long time has passed, and check every time that the client's health has not changed. Permission to use photos can be withdrawn at any moment: in Pigmatia the withdrawal is recorded with its date.
Frequently asked questions
Is a consent form signed on a tablet valid?
It depends on the rules in your country. In general what matters is being able to show who signed, what, and when: that is why Pigmatia produces a PDF with date and signature. For specific requirements, ask your adviser.
Can I use my own wording?
Yes. The consent and privacy texts can be customised, so you can adapt them to the guidance of your adviser or your trade association.
Is consent needed for a touch-up as well?
It is good practice to check the client's health again and have the consent confirmed, especially if some time has passed since the first treatment.
